One of the benefits of the Qest4 technology is that you can have a test done even if you are not physically present! We help many out-of-town clients this way. It is done using energetic DNA found in your hair, nails, urine, and saliva. All of the information, instructions, and forms to begin this process are provided for you below.
Before your first Qest4 test:
Please download, print, and fill out the “Health History” form and send it with your samples.

Please download, print, sign, and bring the form with you to your first appointment.
For each Qest4 test:
Please download the form called “Hair, Nails, Urine, Saliva” for instructions on how to take the samples and mail them. It is important that you contact us to schedule an appointment and discuss pricing before sending your samples. This ensures the samples are tested before they go bad, and it gives us the opportunity to discuss tests available. There is also a membership form required that is only available through e-mail; please remind us to take care of this when you contact us.
The “HNUS Test Requisition” form lists each of the tests available. All the tests in the box called “Standard Scans” are automatically included for each client. If you have specific health issues you wish to address you can request supplemental scans by choosing them from the box titled “Additional Requested Scans.” Please download, print, fill out, and send this form with your samples. Please contact us before sending samples to discuss which tests you would like to have done. Payment must be sent with samples and additional scans have their own pricing.
Please download, print, and fill out a “Follow-Up” form for each subsequent test and mail it with your samples.
